What is Bactrim?

Bactrim is a systemic antibacterial. This product is recommended for the treatment of infections with susceptible germs and in the prevention of different infections. It presents in the form of tablets.

Bactrim should in principle be used only when other available treatments are either ineffective or poorly tolerated. The decision to use Bactrim should take into account, in a given country, the evolution of the susceptibility of the germs to the product and other available antibiotics. Bactrim is indicated from the age of 6 years.

What Is The Right Dosage Of Bactrim?

Swallow Bactrim with a sufficient amount of liquid, preferably after meals. The recommended daily dose in adults 3 to 4 tablets for at least 1 month. This dose requires adjustment according to the age of the patient, the weight, the renal function and the severity of the disease. During acute infection, Bactrim for at least 5 days or continue for 48 hours after the disappearance of clinical symptoms The dosage for children is a daily dose of approximately 30 mg SMZ + 6 mg TMP per kg body weight . During severe infections, this dosage may be increased by half.

What Are The Side Effects Of Bactrim?

As with any other drug, allergic reactions may occur in patients who are hypersensitive to the components of the product: e.g. fever, angioedema, anaphylactoid reactions, serum sickness. Pulmonary infiltrates, such as occur in eosinophilic or allergic alveolitis, have been reported. They can manifest as symptoms such as coughing or shortness of breath. If such symptoms appear or worsen unexpectedly, the patient will be reevaluated and discontinuation of Bactrim therapy will be considered. A high dose of TMP, such as that used in patients with Pneumocystis jirovecii pneumonia, induces a progressive but reversible increase. Serum potassium levels in a significant number of patients. Even at the recommended doses, TMP may produce hyperkalemia when administered to patients with underlying conditions of potassium metabolism or renal failure, or receiving drugs that induce hyperkalemia. Close monitoring of serum potassium is warranted in these patients. Cases of acute pancreatitis have been reported; many of these patients had serious illnesses, including AIDS. Many skin reactions have been described, they are usually mild and quickly reversible after stopping treatment. Sulfonamides, including Bactrim, may induce an increase in diuresis, particularly in patients with cardiac edema.

A skin reaction or other serious adverse reaction requires the immediate cessation of treatment. Bactrim will be used with caution in patients with a history of severe allergy and bronchial asthma. The risk of serious adverse reactions increases in the elderly or when there are risk factors, such as renal and / or hepatic insufficiency, or concomitant use of others. To minimize the risk of side effects, the duration of Bactrim therapy will be as short as possible, especially in the elderly. In case of renal impairment, the dosage will be adjusted according to the specific dosage instructions.

Are There Interactions With Bactrim?

Trimethoprim is a carrier-2 inhibitor of organic cations (OCT2) and a weak inhibitor of CYP2C8. Sulfamethoxazole is a weak inhibitor of CYP2C9. Systemic exposure to drugs transported by OCT2 may increase when co-administered with TMP-SMZ. Examples are dofetilide, amantadine and memantine. TMP-SMZ should not be administered in combination with dofetilide (see section 4.3). There is evidence that TMP inhibits renal excretion of dofetilide. The combination of 160 mg TMP and 800 mg SMZ coadministered twice daily with 500 mg dofetilide results in a 103% increase in dofelitide concentration-time (AUC) area and a 93% increase the maximum concentration (Cmax) to dofetilide. Dofetilide may cause serious ventricular arrhythmias associated with QT interval prolongation including torsades de pointes that are directly related to plasma dofetilide concentration. In patients receiving amantadine or memantine, there may be an increased risk of neurological adverse events such as delirium and myoclonus. Systemic exposure to drugs metabolized primarily by CYP2C8 may increase with co-administration of TMP-SMZ. Examples are paclitaxel, amiodarone, dapsone, repaglinide, rosiglitazone and pioglitazone. Co-administration of clozapine, a drug known for its important potential to cause agranulocytosis, will be avoided. An increase in the incidence of thrombocytopenia was observed in the elderly receiving simultaneously certain diuretics, mainly thiazides. Platelets will be monitored regularly in patients receiving diuretics.
